RFL Participant Waiver Form
Here is the Relay For Life Participation Form, that must be signed by each team member and people who come along to walk on the track.

Instructions
o Each team member should be registered in your team, at cost of $15 each, via
www.relayforlife.org.au or 1300 65 65 85;
o Print or email this form to each team member to fill-in and sign;
o Bring the forms along to the event and submit them at the Check-In window. In return we’ll give you a wrist band for each form;
Note – team members can be added to your team at the event, via this form. We’ll have t-shirts for those people upon registering.

Team Tent Site Allocations
Tent site allocation is assigned on a first in best dressed policy. The earlier you register your team, the higher the priority you have in picking which tent site(s) you would prefer to set up in for the 2008 Relay For Life.
